"I performed my first implant surgery in 1990 with a blade type, so I've placed about 30,000 implants, and I think I've tried almost every type of implant that's been introduced to the market. In the early 2000s, I was so engrossed in clinical research that I applied for 26 patents related to peri-implantitis."

 

Dr Hyun has been practising dental implantology for 33 years and has been practising for about 30 years.


Since the 1990s, he has been directly involved in the entire process of implant surgery and prosthetics, and has followed the development and clinical application trajectory of countless implants, including the effects of implants on the human body, which type of implant is optimal for the patient, and the success and failure factors of implants.


 

In 2000, he had the foresight to file a U.S. patent application for peri-implantitis and a design to prevent it, when no one else was paying attention.

 

30 years of experience in dental implantology 

 

Dr Hyun believes that at least 10 years of follow-up is required to demonstrate the clinical effectiveness of an implant.


 

Based on the research and development and clinical application of these implants, there are three main implants that are currently in the spotlight. The first of these three is the Sterioss implant.

 

What makes an implant successful from a researcher's perspective

 

 What does Dr Hyun think makes an implant successful?


Dr Hyun believes that tissue-level implants will become more popular in the future. Tissue-level implants are able to withstand loads with smaller implants. Unlike bone-level implants, they have a number of advantages over bone-level implants, such as being able to be applied in load-bearing areas, having an overlying application point, and having a one-piece tissue connection.


Nevertheless, the use of Bone Levels in private practice remains strong because of their familiarity.


In addition, the use of smaller implants allows for a much simpler and easier surgical scope and a smaller bone graft volume.


In particular, flapless implantation without opening the flap is possible, and we have recently developed guide pins and have more than 5,000 clinical cases.


He also advised that implant clinics should pay attention to the global aging of the population. Korea has also entered an ultra-elderly society, with 20 per cent of the population aged 65 or older. With this aging population, there is a concern that the scope of surgery will shrink and the risks will increase due to systemic diseases and decreased immunity, so the focus will be on incisionless or minimally invasive methods that can be applied as little as possible.

 

80-90% of implant placements are 1-2 implants

 

 

As a clinical implant researcher, Dr Hyun examined the cases performed at PerioPlant Dental.


One to two implants account for 80 to 90 per cent of the cases. Single cases account for about 55 per cent and two cases for about 45 per cent, meaning that more than three implants are placed at a time in only about one in 10 cases. With such a small number of implants, we need a system that is easy to apply.


That's why we developed the PinGuide system, which can be performed immediately after consultation. In the future, implant surgery will move towards minimally invasive techniques and less bone grafting.

 

Sterioss systems have been around for a long time

 

 

In this respect, the Sterios implant is in line with trends in implant success.


First of all, the implants are 1mm apart and can be selected according to the patient's case. Systems with 4mm implants are rare in the world and Sterioss is the only one in Korea.


Sterioss implants are made up of 1mm increments. With the introduction of the Sterioss implant, it is possible to plant 1mm shorter, so 9mm for the upper jaw and 7mm for the lower jaw are commonly used.


Sterioss implants, which opened up the era of 1mm increments, allow for shorter implants to be supported by the bone, making it possible for patients to avoid the need for burdensome vertical hip surgeries in areas that were difficult to place implants in just four to five years ago.


In particular, the design structure itself is tapered to strengthen the implantation fraction, which enhances initial fixation. The procedure itself is much easier than in the past, as it can be applied to difficult cases or niche cases.


Dr Hyun said that the shorter length of the implant, like the Sterioss implant, is beneficial to the patient because it shortens the healing period, and it is also a breakthrough system for the clinician, who can perform surgery and prosthetics without the burden of surgery.


In addition, Sterioss is a company that has as much expertise in implantology as current clinicians, and it is also an advantage that it actively incorporates advice from clinicians.


The actual surgeon also provided advice on the design of the implant, which helped to maximise the benefits of the Sterioss implant.

 

Advice for young dentists opening a practice

 

 

"I thought the seniors had it much better, just like the juniors think. Looking back, the survival rate and success rate are different when you think about what the success rate of implants will be after 10 years. Success is less than 80 per cent of survival, so if you look at about 800,000 implants a year, about 5 per cent, or 40,000, will fail," he said, adding, "There are a lot of reoperations, and if we rely solely on reservoirs for implant placement, it will disrupt the implant market."


In particular, recently, a patient with 10-15 years of upper and lower full-mouth implants came in for bone grafting with the implants removed in the middle, and the bone had melted down to the point where we had to place 3m-5mm implants. The patient was almost despondent, and it was very sad to see his face deformed and unable to use his dentures properly. In the end, he was treated with maxillary sinus augmentation, but I advised him that dentists need to look at the next 10 years rather than the next 10 years, so we need to approach implant treatment with a sense of mission, not just as a machine that places implants.
